OCCUPANT CONTACT & ACCOUNT CHANGE
Update any account information that has changed. Must be filled out by Occupant as stated in the Rental Agreement or by a person already authorized in-writing by the Occupant to modify account information. A copy of the photo I.D. of the Occupant or authorized person must be attached below where indicated and the document must be signed and dated below in order to take effect.

Only the Occupant is able to change information on an account unless Occupant has already given written authorization to another party to do so or has already given documentation to the Company showing proof of power-of-attorney or as executor of Occupant's estate, etc, or is an authorized person acting on behalf of a corporation, trust, or other such entity.

RENTAL AGREEMENT TERMINATION
In vacating, Occupant shall cause the Unit to be completely cleared and cleaned and to be free of any damages. Then Occupant shall report this to the Company Office, which shall then determine if Company standards for vacating have been met. If Company agent has deemed this so, Occupant shall sign in the section below, which shall cause this Rental Agreement to terminate. If this is not done, the Rental Agreement shall remain effective and rent shall continue to accrue. The rental period is from the first day of the calendar month to the last day of the calendar month. If Occupant is present on the first day of the calendar month, that month’s rent is accrued in full. No pro-rata refunds will be given if Occupant vacates prior to the end of the rental period. OCCUPANT MUST RETURN ALL KEYS ISSUED BY THE COMPANY. If Occupant has been issued a key(s) by Security Deposit from the Company, this key(s) must be returned on the day of Occupant’s signing the Rental Agreement Termination to be remitted Deposit. All keys issued by the Company to Occupant must be returned for Occupant to be remitted the Deposit amount. Any locks provided by the company must be secured in the storage Unit latch. The cost of any missing lock or locks will be charged to Occupant and/or deducted from the Deposit amount. Occupant is solely responsible for completing the move-out process. Any personal property of Occupant which shall remain in or on the premises after the termination of the Rental Agreement shall be considered abandoned; Occupant will be charged a $50 minimum cleaning fee and any expenses incurred. The foregoing will not affect any other outstanding Rental Agreements or other agreements that Occupant may be entered into with Guardian Storage, Inc.

I authorize the above named person to complete the move-out process, close the account, receive the key deposit or any other monies in my stead. This person will be required to present a photo I.D.
